Medical Records Transfer

Your medical records are confidential and protected by federal privacy and security regulations. To authorize the release of your records, please complete and sign the HIPAA-compliant authorization form available for download here. Please submit the completed form by emailing it to SBKMedicalRecords@vituity.com as soon as possible.
